Bones form the skeleton of the body and allow the body to be supported against gravity and to move and function in the world. Bones also protect some body parts, and the bone marrow is the production center for blood products.

Bone is not a stagnant organ. It is the body’s reservoir of calcium and is always undergoing change under the influence of hormones. Parathyroid hormone increases blood calcium levels by leeching calcium from bone, while calcitonin has the opposite effect, allowing bone to accept calcium from the blood.

What causes a fracture?

When outside forces are applied to bone it has the potential to fail. Fractures occur when bone cannot withstand those outside forces. Fracture, break, or crack all mean the same thing. One term is not better or worse than another. The integrity of the bone has been lost and the bone structure fails.

Often a fracture is easy to detect because there is obvious deformity. However, at times it is not easily diagnosed. It is important for the physician to take a history of the injury to decide what potential problems might exist. Moreover, bone fractures don’t always occur in isolation, and there may be associated injuries that need to be addressed.

Fractures can occur because of direct blows, twisting injuries, or falls. The type of forces on the bone may determine what type of injury that occurs. Descriptions of fractures can be confusing. They are based on:

where in the bone the break has occurred,

how the bone fragments are aligned, and

whether any complications exist.
The first step in describing a fracture is whether it is open or closed. If the skin over the break is disrupted, then an open fracture exists. The skin can be cut, torn, or abraded (scraped), but if the skin’s integrity is damaged, the potential for an infection to get into the bone exists. Since the fracture site in the bone communicates with the outside world, these injuries need to be cleaned out aggressively and many times require anesthesia in the operating room to do the job effectively.

Next, there needs to be a description of the fracture line. Does the fracture line go across the bone (transverse), at an angle (oblique) or does it spiral? Is the fracture in two pieces or is it comminuted, in multiple pieces?

Finally, the fracture’s alignment is described as to whether the fracture fragments are displaced or in their normal anatomic position. If the bones fragments aren’t in the right place, they need to be reduced or placed back into their normal alignment.

Belmont Stakes prediction

I have to admit that I am extremely disappointed that Rachel Alexandra is not running in Saturdays Belmont Stakes. As anyone who has read my past blogs on horse racing knows, I have been trumpeting this horse as one of the truly great horses of our era and a filly to boot.  Its a major loss not having her in the Kentucky or the Belmont, but we do have an exciting race on our hands with Calvin Borels chance to win a sort of triple crown for jockeys.

I have been reading alot about this race and this quote from Borel really made me think about a wager on Mine That Bird. On Monday, five days before the Belmont Stakes, jockey Calvin Borel hopped off Mine That Bird after a workout and had this to say about the Kentucky Derby winner and Preakness runner-up’s chances at Saturday’s Belmont Stakes: “We’re gonna win, no questions asked,” he said.

I loved watching Mine that Bird even in the Preakness and I thought she may have had a chance to catch Rachel Alexandra at a longer distance. He has been fantastic in the Kentucky and the Preakness, so his Kentucky Derby victory is looking less like a fluke and he looks more like a serious horse to contend with. He should love the distance of the Belmont, and His sire, Birdstone, won the 2004 Belmont.

two other horses to watch are Charitable man and Dunkirk. Charitable Man on the Peter Pan four weeks ago in impressive fashion. Charitable Man is a perfect 3-for-3 on dirt surfaces and 2-for-2 at Belmont.

Dunkirk had a bad Kentucky Derby but he did stumble and did not like the sloppy conditions of the Derby according to his trainer. I like Dunkirk but I lost some money on this impressive horse at the Kentucky so I am a little gun shy.

I am gong to have to go small on Mine That Bird to place.

The young Oklahoma City Thunder have emerged as a potential contender for a playoff spot in the Western Conference, but the forecast is anything but positive for them tonight in the Windy City.

The Thunder ended 2009 with five consecutive wins to put the franchise four games over .500 for the first time since Ray Allen and Rashard Lewis led the then-Seattle Supersonics to 52 wins in the 2004/05 season.

However, that run came to a halt in their first game of the new year, as they fell 103-97 in overtime at Milwaukee on Saturday. The Thunder overcame an early double-digit deficit to take a 60-50 lead early in the third quarter, but the Bucks rallied and took the win.

The Bulls ended 2009 on a nice run as well, and it was a much-needed nice run, as it’s taken a lot of heat off of under-fire coach Vinny Del Negro, who looked to be all set to go in front of the firing squad just a week or so ago.

However, the Bulls have strung together four wins in a row, and all of a sudden, Chicago is the 7th slot in the top-heavy East, and Del Negro can sleep a little easier at night (for now, at least). Their most recent win was the most impressive of their run, as they knocked off Orlando 101-93 in a game in which they led by as many as 15 before the Magic rallied to cut it to three in the final half minute.

The Bulls have been better at home (11-6) than on the road (3-11) throughout the season, and they come into tonight having won three in a row and five of six at home. That success is thanks in no small part to second-year point guard Derrick Rose, who is averaging 24.3 points, 6.8 assists, and 4.7 rebounds in his last six home games and has two 30-point games in that stretch (32 in a win vs. Atlanta and 30 vs. Orlando).

Every Bulls win pushes Del Negro farther and farther away from the unemployment line, and the Bulls will duly deliver a fifth straight win tonight.
